Sports Representative Application Information

Each year Aquinas College relies on receiving information from students to ensure awards and trophies are distributed to worthy recipients at the Sports Prizegiving held Annually in late October/early November.

All information will contribute towards the administration of our Sports Awards.

  • Please provide information for all achievements between October 2017 – September 2018.
  • If you would like a hard copy of this form, please collect one from the Sports Office.
  • It is important that this form is returned to the Sports Office, by Wednesday 12 September 2018

The judging panel do their best to understand the internal structures of the many sports organisations/codes our students are involved in.  However, as sports codes vary in their processes for various selections of their teams/groups and which events teams compete at, it is really important that when you complete the columns you are placing information in the correct column/s.

We ask that your information is as accurate as possible – organisation names, team names, age groups, competitions/tournaments.
